To help hospitals navigate CJR and bundled payment initiatives, Stryker Performance Solutions (SPS) launched JointCOACH™, a digital engagement and education platform to coordinate care delivery and improve the experience of patients undergoing joint replacement.
JointCOACH will be offered through Stryker’s Destination Centers of Superior Performance, comprising >270 U.S. hospitals undertaking “care redesign” programs.
The JointCOACH web-based system supports patient communication with a hospital care team during the entire episode of care, from the time surgery is scheduled until at least 90 days after discharge. Pre-op preparation, clinical protocols, information about medications and pain control, recovery and rehabilitation activities are delivered throughout the process, including questionnaires and surveys to keep the patient engaged and involved.
