Study results indicate that use of Kinamed’s CarboJet® CO2 Bone Preparation System yields increased bone cement penetration in total knee arthroplasty (TKA), even in procedures in which a thigh tourniquet was not used to control bleeding.
Study authors noted that tourniquetless TKA is re-emerging due to potential pain control benefits, which is timely given the opioid crisis and high volume of knee replacement procedures. Optimal cement technique and implant fixation are critical to long-term success of cemented TKA, as aseptic loosening is one of the primary causes for early and late revisions. Increasing cement penetration has been shown to provide a stronger bone/cement interface, leading to increased stability and long-term implant survivorship.
The study compared cement penetration in three patient groups:
- Tourniquet without CO2 gas bone preparation
- No tourniquet with CO2 gas bone preparation
- Tourniquet with CO2 gas bone preparation
Results showed that when sterile compressed CO2 gas is used for bone preparation, cement penetration is increased in not only the tourniquet (Group 3) but also in the no-tourniquet group (Group 2), as compared to the tourniquet group in which CO2 gas was not used (Group 1).
CarboJet’s CO2 gas jet cleans and dries the bone bed by bringing blood, saline and lipids/fatty marrow elements to the surface for collection and removal. The process takes no more time than is typically required for drying with lap sponges. The benefit is improved cement penetration depth, offering the potential for a reduction in bone/cement interface stress and increased cement mantle toughness.
